Hospital Discharge Report

We worked in partnership with Wye Valley NHS Trust to engage with people about a wide range of aspects relating to hospital discharge.

Summary

  •  Healthwatch Herefordshire collaborated with Wye Valley Trust to help improve patient experience.
  • Engagement took place between October 2020 and February 2021
  • We collected 61 patient stories which were made up of 58 telephone interviews and 3 via email.
  • Key findings focused on topics including medication; the discharge lounge; transport; communication; aftercare and carers.
