Focus on Community Health Services

The Herefordshire Clinical Commissioning Group (The people who plan and buy Health services in the County) & Healthwatch are jointly hosting a feedback session on Community Health Services.
Come and share your views on how you would like community health & care services to look in the future.
We need your views in order to influence how services are planned and delivered in Herefordshire.
Our plans for the event will be:
Guest speakers Dr Mike Hearne - Medical Director of Taurus Healthcare and Jade Brooks - Deputy Director of Operations, will be presenting a new model of working for community health services called 'Primary Care Home'. Come and hear what this means for patients.
We will be holding a workshop exercise where you can tell us your views on: Healthcare in the community such as; GP services, Mental health care, care at home, district nursing, therapies, reablement, care after discharge from hospital, community hospitals and clinics in community hospitals. Social care in your community, care homes, care at home, support from charities and voluntary sector organisations.
We will follow this with an opportunity for you to have an open question and answer session with Healthwatch
Ending the evening by holding our brief board meeting in public.
